Program Code of Conduct

1. Everyone is expected to participate in all planned activities as defined by the program schedule.

2. Obscene and discriminatory language, insubordination and/or put downs will not be tolerated at any time.

Programs are meant to be fun, educational and recreational. For the benefit of all participants, it is important that children behave appropriately. If it becomes necessary to take disciplinary action against a participant, the following steps will be taken:

1st Incident: The participant will receive a verbal warning and an explanation as to why the behavior is inappropriate

2nd Incident: Staff will determine an appropriate consequence for the youth's actions

3rd Incident: The child will be excused from the program

Waiver-

PEACE Inc. Has the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of the minor to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration.

This authorization shall continue indefinitely, unless I otherwise revoke this authorization in writing, or the minor(s) revokes this authorization in writing upon reaching majority age.

I waive any right that I or the minor(s) may have to inspect or approve any finished product in which the minor’s likeness appears, including written or electronic copy.

I waive any right to royalties or other compensation arising or related to the use of the photograph.

I hereby hold harmless and release PEACE Inc. from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate and the minor(s) estate(s).

I warrant that I am of full age and have every right to contract for the minor(s) in this regard. I state further that I have read the above authorization, release, and agreement, prior to its execution, and that I am fully familiar with its contents. By executing this Release and Parent Permission Slip and granting the permission stated herein, I, for myself, heirs, personal representatives and/or assigns, hereby release PEACE Inc. and its respective officers, directors, agents, employees from and against any liability, damages, claims or causes of actions arising out of my child’s participation in this trip, except as otherwise provided by law. I also agree to indemnify and hold harmless to PEACE Inc. from any claims, causes of action, or other judicial proceedings, costs, expenses, damages and liabilities, including attorneys’ fees, brought solely as a result of my child’s negligence, willful misconduct, and/or failure to adhere. I have read this Release and Parent Permission Slip and understand its terms.

This release shall be binding upon the minor(s) and me, and our respective heirs, legal representatives, and assigns.
